BCBSNE is happy to offer four work designations for our Omaha area employees: 100% in-office, Hybrid â?? 3 days in office, Hybrid â?? 2 days in office, 100% remote. If choosing to work remote, you can live within the state of Nebraska or Local Omaha Area.
The community health worker (CHW) will support and coach at-risk patients to manage their own health and assist them in navigating the health care and social service systems. Community Health Workers (CHWs) bridge the gap between health care providers and people in need of social care. They serve as a link between the patient and the health or social service agencies.
What youâ??ll do:
- Assist patients in the communityâ??through home visits, attending medical appointments, and accessing other community-based resources.
- Be onsite at assigned provider or hospital system and build relationships with site staff.
- Review referral forms, needs assessment screeners, or Electronic Medical Record data and determine appropriateness for program participation in conjunction with Care Management team, and provide active referrals to other community resources.
- Complete all telephone and face to face contacts with members in a timely and professional manner.
- Complete enrollment and other needed documentation and document in approved format in a timely manner.
- Complete client-centered treatment goals in collaboration with members and review progress in meeting goals routinely and at identified check-in times.
- Assist member to schedule medical and social service appointments and attend appointments as needed or requested.
- Assist with medical and behavioral health referrals to ensure client access and follow-through.
- Ensure memberâ??s access to interpreters and translators for medical and social service appointments.
- Assist member in securing transportation to medical and social service appointments.
- Assist member in obtaining necessary medications through member assistance programs and other reduced cost programs.
- Assist members in accessing and understanding necessary health and wellness information provided by their medical provider and clinic.
- Provide support and education to members to assist them in adhering to medical recommendations and taking medications as prescribed.
- Provide regular and timely updates to medical providers, referral sources, and assigned clinics on membersâ?? progress to meet treatment goals and to trouble shoot any problems or concerns.
- Complete all required documentation and data tracking in a detailed and timely manner.
- Attend and actively participate in all trainings and meetings.
- Supports quality management and program evaluation activities through ongoing quality improvement process engagement.
Assist with all other duties assigned.
To be considered for this position, you must have:
- High School diploma or GED required.
- Willingness and ability to travel to BCBSNE member homes and community-based settings in NE.
- Experience working with underserved and diverse populations in community settings.
- Experience in navigating health and social service systems.
- The ability to support, coach and motivate patients to reach their goals.
- Good interpersonal skills with ability to relate to diverse groups of people and people on all levels.
- The ability to meet or exceed the attendance and timeliness requirements of their departments.
- Ability to prioritize workload and manage multiple tasks with different deadlines.
- Experience connecting geriatric members with resources in the community is preferred
The strongest candidates for this position will also possess:
- The ability to speak other languages.
- Additional certifications and education in community health.
- Associates or bachelorâ??s degree in healthcare field.
- CPR certification.
- Strong PC proficiency and MS Office skills.
