Pharmacy Technician (Inpatient) - Transitions of Care Sr Associate

Position Highlights:

Nebraska Medicine is now taking applications for a talented individual to join us as an Pharmacy Technician (Inpatient) - Transitions of Care Sr Associate. This role will be great for someone that has a passion for pharmacy and great customer service for our patients we serve. This specific position will work from the main medical campus in Omaha, NE.

The Pharmacy Technician (Inpatient) - Transitions of Care Sr Associate role will be for experienced pharmacy technicians that complete medication histories for patients admitted into the hospital on the main campus. These positions offer a lot of opportunity for patient interaction and utilization of medication knowledge.

We have a small team of highly talented technicians who spend their day directly interacting with patients- so great communication skills are key! Our TOC Sr Associates interview patients/families, call pharmacies and facilities and work closely with our pharmacists to update home medication lists in order to ensure patient safety during hospital admission. These technicians often find and report important information about how patients are taking their medications, alert us to problems patients may be having with side effects or affording their medications, and help the pharmacists bring important medication information to the medical teams. These are very independent roles with opportunity to learn more about medications and pharmacy. This position is for the acute care floors on main campus at Nebraska Medicine.

Shift:

  • Full Time and benefits eligible (0.9 FTE / 36 hours per week)

  • Second Shift Thursday-Wednesday, 11:00 - 21:30

  • This position will work holidays if it is the individual's "on" week.

Pharmacy Technician (Inpatient) - Transitions of Care Sr Associate:

Facilitate and coordinate patient care transitions at admission, transfer, and discharge from the facility. Utilize available electronic resources, patient interview, and phone follow up to ensure an accurate medication history is available for patients at admission or upon presentation to the emergency department. Work in coordination with and under the supervision of a registered pharmacist to ensure accurate medication reconciliation is performed per policy MM07. Review transfer orders and ensure necessary medications have been transferred with the patient. Collaborate with inpatient pharmacists, inpatient nursing, pharmacy financial counselors, and the meds to beds outpatient pharmacy staff to assist in ensuring patient discharge needs are met in a timely manner.

Required Qualifications:

  • Minimum of three years of experience in an inpatient or outpatient pharmacy or other applicable health care setting required.

  • High school diploma or equivalent required.

  • Effective communication skills, initiative, teamwork and flexibility required.

  • Registered as a pharmacy technician with the State of Nebraska Board of Pharmacy prior to start date required.

  • Pharmacy Technician Certification within twelve months of registration date required.

  • Basic Life Support certification within 90 days of transfer or hire required.


Preferred Qualifications:

  • Thorough understanding of available electronic resources to obtain medication history information preferred.

  • Thorough understanding of medical terminology preferred.

  • Thorough knowledge of top 100 medications preferred.

  • Associates or bachelor's degree preferred.

  • Proficient in Microsoft Office preferred.
