Essential Duties and Responsibilities Medical Record Review and Analysis
Conduct thorough reviews of patient medical records to identify gaps in care, potential risks, and areas for improvement
Evaluate treatment plans for alignment with best practices and evidence-based guidelines
Document findings and actionable recommendations clearly and concisely
Improvement Plan Development
Develop and implement quality improvement initiatives to enhance patient care and operational processes using StayPut data
Collaborate with regional and market teams to identify and address areas for improvement
Focus on chronic condition management within patients' current care environments
Clinical Decision-Making Support
Provide expert guidance to caregivers and staff to prevent avoidable hospital admissions and ED visits
Address barriers to effective care delivery through targeted interventions
Education and Training
Offer clinical education and quality improvement coaching to clinicians in the Call Solution team
Educate nursing and care staff on best practices and early intervention techniques
Provide ongoing onboarding and education for clinical team members
Interdisciplinary Collaboration
Work closely with physicians, nurses, social workers, and other team members to ensure comprehensive patient care
Participate in interdisciplinary team meetings to review patient progress, care strategies, and program goals
Program Development and Evaluation
Contribute to refining StayPut protocols and workflows to enhance patient outcomes
Develop and maintain StayPut resources to promote the program within partner facilities
Analyze program metrics to evaluate effectiveness and identify areas for improvement
Compliance and Quality Assurance
Ensure adherence to regulatory and clinical standards
Participate in audits and quality assurance initiatives as needed
Qualifications and Experience
Education: Master's Degree in nursing or Master's Degree in physician assistant studies
Licensure: Active nurse practitioner (NP) or physician assistant (PA) license
Experience: Minimum 3 years' experience as NP or PA in post-acute care or quality improvement initiatives
Skills
Strong knowledge of quality improvement methodologies and tools
Excellent communication and interpersonal skills
Ability to work collaboratively with multidisciplinary teams
Strong analytical and problem-solving abilities
Proficiency in data analysis and reporting
