Documentation of 2 MMRs at least 28 days apart and/or positive laboratory titer
Completed onsite. Completed annually.
Documentation of 2 doses of varicella 4 weeks apart and/or positive laboratory titer
Proof of having received vaccine. The TD vaccine does not meet this requirement.
Physical Capacity. Written statement by a person lawfully authorized to perform the examination that Resource is physically capable of performing Health the essential functions of the job. This health exam must be performed within one (1) year prior to assignment to a Customer.
